Identification consists of how the product is billed (billing units,package size designations), product identification systems, and any type of descriptive data which serves to uniquely identify a product with the intent to establish standards for product identification such that there is no ambiguity in distinguishing one product from another. Meeting Minutes More Information http://url Members Only Button WG10 Professional Pharmacy Services Work Group 10 Professional Pharmacy Services supports the pharmacist’s individual and collaborative planning, delivery, documentation and quality assessment of patient care services through the development and distribution of standards, standards-based templates, and implementation guides. These documents/tools in conjunction with the publication of best practices recommendations encourage patient engagement, foster patient safety and support consistent delivery of patient care services to optimize outcomes. WG10 strives to support pharmacists in utilizing their unique training and expertise to provide patient care services related to the appropriate use of medications. Patient care services are provided in many different practice settings and include activities such as medication therapy management, clinical reconciliation (medication, allergies and problems), patient immunization management, disease state monitoring, adverse drug event reporting and therapy adherence program management. Pharmacists contribute to improving patients’ health by providing patient care services as authorized by license and allowed under their scope of practice and collaborative practice agreements. Meeting Minutes More Information http://url WG10 Professional Pharmacy Services Goals Encourage the integration of the Joint Commission of Pharmacy Practitioners Pharmacists’ Patient Care Process into pharmacy practice and pharmacy systems workflow by development or modification of administrative and clinical standards to provide the tools needed to: Collect and assess patient information, Develop a patient centered care plan that addresses medication-related problems and optimizes medication therapy and patient engagement, Implement the care plan, and Monitor and evaluate the effectiveness of the care plan. Identify clinical issues, both medication and practice related, that will require standards development and/or enhancement. The standards changes will be developed either by WG10, or in conjunction with other WGs, other Standards Development Organizations (SDOs) and other standards setting organizations. Review Data Element Request Forms (DERFs) and adjudicate ballots pertaining to standards for which this work group is responsible. Develop standards and implementation guides that enhance the patient-centered care process through the use of interoperable information technology that facilitates communication among all individuals involved in the patient’s care. Identify segments of pharmacy practice, such as compounding, specialty, home infusion, etc., that may have unmet communication and information exchange needs and collaborate with other NCPDP Work Groups in the development or enhancements of standards, guidance and terminologies to address those needs. Monitor and respond to federal and state legislation/regulation related to pharmacists providing patient care services and identify potential changes needed to data exchange and interoperability standards to support new legislative requirements. Educate NCPDP staff and membership on health information technology (HIT) implications of federal and state legislation/regulation related to pharmacists providing patient care services. Identify gaps in the exchange of clinical information, such as vocabularies or coding systems, and work to facilitate solutions. In conjunction with NCPDP staff, work with colleges of pharmacy, and/or their associations, to identify opportunities to incorporate information about HIT and data exchange standards into the pharmacy curriculum.
